In emergency or crisis situations, Dr. Victoria may appear at your scheduling meeting time between 10 to 15 minutes late. You may be notified of a situation prior to your appointment time, but if you are not, please exercise patience and understanding - Dr. Victoria will be with you shortly.
A comprehensive ADHD Evaluation is charged at a minimum of $2,000.00. This fee includes costs for the hours of testing services, scoring, report-writing, and the feedback session. Additional fees may apply, and are not limited to: expedited reports, additional or extended feedback sessions, and/or legal procedures.
An initial session is charged at $300.00. This fee includes a Diagnostic Interview (DI), conducted to gather history, discuss concerns, symptoms, therapy goals, testing, the therapeutic process, and a treatment plan.
An Individual Therapy session is charged at $250.00 per session.
You should carefully read the section in your insurance coverage booklet that describes mental health services. If you have questions about the coverage, call your plan administrator. Of course, I will provide you with any information I can based on my experience.
You are encouraged to contact your insurance company to determine if your plan provides coverage benefits before your first appointment. It is important to understand that you are responsible for determining what your insurance will cover, including relevant deductibles and requirements for pre-authorization, if needed.
You should also be aware that most insurance companies require that I provide them with your clinical diagnosis. I may be required to provide additional clinical information, or copies of the entire record. Though all insurance companies claim to keep such information confidential, I have no control over what they do with it is in their hands. In some cases, they may share the information with a national medical information database. I will provide you with a copy of any records I submit, upon request. You understand that, by using your insurance, you authorize me to release such information to your insurance company.
If your account has not been paid for more than 60 days, and arrangements for payments have not been agreed upon, I have the option of using legal means to secure the payment.
I am not immediately available by telephone. When I am unavailable, my telephone will be answered by an answering service. I will make every effort to return your call, or have a member of my staff return your call, within 48 hours. This policy does not apply when I am not in the office; under this circumstance, my answering service will indicate when I will be available again. If you are difficult to reach, please inform me of some times when you will be available. In the event of an emergency, please call 911 or go to the nearest emergency room.
Due to the importance of your confidentiality and of minimizing dual relationships, I do not accept friend requests or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, Instagram, Twitter, etc.). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have any questions about this, please bring them up to me when we meet and we can talk more about it.
Florida state law requires that all custodial parents give consent for the treatment of a minor. Therefore, I will be unable to provide psychotherapy to a child under the age of 18 years if consent cannot be secured from all custodial parents.
If you share legal custody and your divorce decree notes that you must inform the other parent of health appointments. Please note that psychotherapy and evaluation services fall under this, and may be in violation of a court order if you fail to inform the other parent of our services with your child. Also, understand that to provide consent for your child, you must either have sole legal custody OR have shared legal custody. If you have no legal custody, you cannot provide consent on services. I will need authorization from both legal guardians and/or a copy of any legal agreements detailing this consent before beginning treatment.
If you are under the age of 18, please be aware that the law may provide your parents/guardians the right to examine your record. I will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
The most secure form of communication is by face-to-face, telehealth consultation, phone, or voicemail. Email communication is utilized only with your permission and only for administrative purposes, unless we have made another agreement. Email exchanges should be limited to things such as appointment reminders, billing matters, and other related issues. I cannot ensure the confidentiality of any form of communication through electronic media, including text messages. If you prefer to communicate via email or text messages, I cannot guarantee immediate response and request that you do not use methods of communication to discuss therapeutic content and/or request assistance for emergencies.
Services by electronic means, but not limited to, telephone communication, the Internet, and email is considered telemedicine. Telemedicine is broadly defined as the use of information technology to deliver medical services and information from one location to another.

Written records are released only after a consent form is signed by the client or their Parent/Legal Guardian.
To adhere to ethical and legal standards, I will refrain from releasing data to protect you or others from substantial harm, misuse, or misrepresentation.
Ending relationships can be difficulty. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of treatment. I may terminate after appropriate discussion with you, if I determine that the psychotherapy is not being effectively utilized, or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If therapy is terminated for any reason, or you request another therapist, I will provide you a list of qualified psychotherapists to treat you. You may also choose someone else on your own or from another referral source.
Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.
